Loma Support Services , we believe independence looks different for everyone. We specialize in Tailored Day Services (TDS) , providing individualized, one-on-one support throughout the community. The name Loma means hill in Spanish. It is to walk alongside them, provide the right level of support, and help them become increasingly confident and independent. We are currently looking for thoughtful, reliable, and motivated Community Facilitators to join our growing team. Growing With Loma Loma Support Services is a newer, growing organization, and we're building our team thoughtfully from the beginning. We staff each participant individually based on their goals, needs, schedule, location, and staff fit. Because of this, hours may begin gradually and increase as additional participant matches become available. We're looking for people who are comfortable joining a growing organization and who are interested in growing with us as we continue to expand. What This Role Looks Like This is not a traditional facility-based day program position. As a Community Facilitator, you will work one-on-one with a participant in real community settings and help them make progress toward individualized goals. Every participant is different, so your work may look different from day to day.

Exploring employment interests and career options Job readiness and employment-related skills Finding volunteer opportunities College or other post-secondary education Learning how to navigate the community Transportation and mobility skills Self-advocacy and communication Money management and budgeting Grocery shopping and meal planning Accessing community resources Social and recreational activities Building confidence in unfamiliar environments Learning how to plan and organize their day Developing greater independence with everyday responsibilities These areas align with the individualized supports Loma currently identifies in its Tailored Day Services program. You will also be responsible for documenting services, communicating appropriately with the Loma team, and keeping your work focused on the participant's identified goals. What Makes Tailored Day Services Different Tailored Day Services are exactly that: tailored . Participants are not placed into the same schedule or expected to work on the same activities. Instead, services are built around the individual's interests, strengths, needs, and goals. Some participants may be focused heavily on employment. Another may be learning transportation skills. Another may be attending college, becoming more socially connected, or learning how to advocate for themselves. Loma's TDS program provides one-on-one support in areas including employment, community integration, transportation, recreation, self-advocacy, and post-secondary education. The Kind of Staff Member We're Looking For We are not simply looking for someone to accompany a participant throughout the day. We are looking for someone who knows how to support, teach, encourage, and gradually build independence . A strong Community Facilitator understands when to step in, when to prompt, and when to allow someone the opportunity to try something independently.

Transportation Because Tailored Day Services take place throughout the community, some participant assignments require staff transportation. For positions requiring participant transportation, you must have: A reliable personal vehicle available for participant transportation A valid California driver's license Current automobile insurance An acceptable driving record A vehicle that meets required safety and inspection requirements Transportation may include driving participants to community activities, educational programs, volunteer opportunities, appointments, employment-related activities, or other locations connected to their individual goals. Mileage reimbursement is provided for eligible work-related mileage. Requirements High school diploma or equivalent Experience supporting individuals with developmental disabilities preferred Ability to work independently in community-based settings Reliable attendance Ability to complete required documentation and progress notes Ability to complete required background and pre-employment screening CPR and First Aid certification, or willingness to obtain certification Reliable personal transportation for assignments requiring participant transportation Valid California driver's license and current automobile insurance when transporting participants Why Loma?
