San Fernando, CA Job Details $45 - $55 an hour 18 hours ago Qualifications CPR Certification Licensed Clinical Social Worker Driver's License Full Job Description Job description : Access TLC Health Care is currently seeking dedicated and caring Medical Social Worker to join our Home Health and Hospice department. The Medical Social Worker is responsible for assessing patient's illness and environment relating to home health services, planning intervention based on evaluation findings and providing information and referral services for patients and families/caregivers. All charting is done electronically, and we will provide you with an internet enabled iPad.
Per Diem Pay Rates:
$55.00 per visit: 40 minutes or more of visit time $45.00 per visit: 39 minutes or less of visit time
Meetings:
$20.00 per hour for IDT meetings, in-services, and staff meetings
Phone Calls:
$20.00 per hour for patient-related calls. Calls made solely to schedule patient visits are included in the visit rate. Access TLC is an accredited hospice and home health agency who provides unparalleled quality of service that allows patients to have a better quality of life. We have been serving Ventura, Santa Barbara, and Los Angeles counties for over 22 years. Requirements include : Licensed as a licensed clinical social worker by the Board of Behavioral Sciences. Minimum of one year's experience in health care. Spanish speaking preferred. CPR Certification. Must have a California Driver's License, satisfactory driving record and a vehicle that is insured in accordance with state/or organization requirements.
