In this vital role, you will provide essential support to clients in their homes or care facilities, promoting independence, safety, and well-being. Your energy, empathy, and attention to detail will help create a nurturing environment where clients feel valued and cared for every day. This paid position offers an opportunity to develop your caregiving skills while making a positive impact in the community. Responsibilities Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using patient transfer methods and equipment such as Hoyer lifts. Monitor patient health by observing vital signs, recognizing changes in condition, and reporting findings accurately using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems. Administer medications and perform catheter care following home healthcare regulations and medical terminology standards. Support clients with dementia or Alzheimer's disease through specialized memory care techniques and compassionate engagement. Provide companionship and emotional support while maintaining patient confidentiality in accordance with HIPAA guidelines. Assist with light housekeeping tasks such as cleaning, laundry, meal preparation, and ensuring a safe environment for clients with mobility challenges or disabilities. Document medical records and care activities thoroughly to ensure compliance with home health agency standards and regulatory requirements. Experience Prior experience working in home care setting is highly preferred. Demonstrated knowledge of home health care assistance, geriatrics, dementia care, and end-of-life care practices. Experience working with individuals with Alzheimer's and Dementia Strong understanding of home & community care regulations to ensure all services are delivered safely and compliantly.
