POSITION DESCRIPTION

MEDICAL SOCIAL WORKER

Coordinates client care with Case Managing RN, team members, and Clinical Supervisor. Evaluates client medical social needs and develops plan of care/goals appropriate to findings. Participates in in-services, meetings, and case conferences as requested. Instructs client and family in community resources and makes appropriate referrals. Revises and updates medical social work plan of care as appropriate.

QUALIFICATIONS

Competent in the provision of medical social work, including evaluation and assessment. Knowledge of community resources. Current CPR training/certification Graduate degree in social work from an accredited school of social work and current licensure in the State of Colorado. 1 year minimum post degree experience as medical social worker with home care experience preferred.

$45.00 - $70.00 per hour
