Responsibilities In need of HHA for live-out weekend Saturday and Sunday. 8hours per shift. start Asap. Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using tools like Hoyer lifts when necessary Monitor patient health by observing vital signs, reporting changes, and maintaining accurate medical documentation in Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems Support medication administration following prescribed protocols while ensuring compliance with HIPAA regulations for patient privacy and confidentiality Provide specialized care for individuals with dementia or Alzheimer's disease, including memory care techniques and behavioral management strategies Perform light housekeeping tasks such as cleaning, laundry, meal preparation, and ensuring a safe environment for patients Assist with medical procedures such as catheter care, wound dressing changes, and hospice-related support under supervision Document patient progress meticulously in medical records and communicate effectively with healthcare teams to coordinate ongoing care plans Experience Prior experience working in assisted living facilities, nursing homes, or home health environments preferred Demonstrated ability to work with individuals with developmental disabilities or disabilities requiring specialized care Knowledge of medical terminology and familiarity with medical documentation processes Certification in CPR and first aid required; additional certifications such as hospice care or dementia training are a plus Experience operating EMR/EHR systems and managing patient records securely under HIPAA guidelines Long-term care experience and home & community care background highly valued Strong communication skills, patience, empathy, and the ability to handle sensitive situations professionally Join us to empower those in need through compassionate caregiving!
