Job Summary We are seeking a compassionate and dedicated Caregiver/Personal Care Assistant to join our team and provide exceptional support to individuals in various care settings. This role involves assisting clients with daily activities, ensuring their safety, and promoting independence through personalized care. Duties Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and toileting to promote comfort and independence. Monitor patients' health status by observing vital signs, mental state, and overall condition, ensuring timely reporting of any concerns. Support medication administration following care plans while adhering to HIPAA regulations to maintain confidentiality. Prepare nutritious meals and assist with meal planning to meet dietary needs and preferences. Provide transportation assistance for appointments, errands, or community outings to foster social engagement. Use proper patient transfer methods and manual handling techniques, including Hoyer lifts when necessary, to ensure safe mobility. Maintain detailed documentation of patient care activities, observations, and progress in compliance with healthcare standards. Qualifications Prior experience working with individuals with developmental disabilities, neurodevelopmental disorders, or in senior/geriatrics care is highly desirable. Knowledge of patient observation techniques, dementia care (including Alzheimer's and memory care), hospice care, and assisted living environments. Certification in First Aid and CPR is required; additional certifications such as medication administration or HIPAA training are a plus. Familiarity with patient monitoring tools like vital signs measurement and resident's (personal health records) management. Experience operating patient transfer equipment such as Hoyer lifts and understanding of patient transfer methods for heavy lifting safety. Strong communication skills for effective client interaction and team collaboration. Ability to perform light cleaning duties related to patient hygiene and environment maintenance; experience in home health or home & community care is advantageous.
