Shifts Vary Position Responsibilities:

Follows the plan of care to provide care/service to the client Helps client to maintain personal hygiene Assist the client with ambulation as approved Plans & prepares meals Assist client with becoming independent according to the care plan Promote client's mental alertness through involvement in activities of interest Reports change in the client's mental or physical condition or in the home situation to the supervisor Performs routine housekeeping tasks as related to client's environment Prepares a visit report and client record Works with personnel of other community agencies involved in the clients care Attends in-service as required by regulation Confer with the supervisor about client's status and care, participate in the planning of service

Qualifications and Requirements :

High School Diploma/GED, preferred Must have access to transportation Knowledge of housekeeping and meal preparation CPR and first aid certification Verbal and written communication skills Ability to bend, stoop, reach and lift up to 30 lbs. occasionally Employer conducts competency test Employer conducts a background and reference check.
