This vital role involves providing attentive, respectful, and comprehensive support to clients across various stages of life and health conditions. As a Home Health Aide, you will work closely with healthcare professionals and families to promote independence, comfort, and well-being, ensuring each client receives the highest quality of home healthcare assistance. Your dedication will help foster a safe, supportive environment where clients can thrive in familiar surroundings. Duties Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support using proper patient transfer methods and equipment such as Hoyer lifts. Monitor patient health by observing vital signs, recognizing changes in condition, and reporting findings accurately using Electronic Health Record (EHR) systems or Medical documentation tools while maintaining strict patient confidentiality in compliance with HIPAA regulations. Support medication administration under supervision, ensuring proper dosage and timing while adhering to home healthcare regulations. Provide specialized care for clients with dementia, Alzheimer's disease, or other memory-related conditions through tailored memory care techniques and compassionate engagement. Assist with medical procedures such as catheter care, wound care, and end-of-life support in hospice or palliative settings. Prepare nutritious meals and assist with light housekeeping tasks including cleaning and laundry to promote a safe and comfortable living environment. Communicate effectively with clients, families, and healthcare teams to coordinate care plans and ensure client needs are met promptly and professionally. Qualifications Proven experience working as a Home Health Aide or in assisted living, nursing homes, or long-term care settings. Demonstrated ability to perform patient observation accurately and respond appropriately to changes in condition. Familiarity with EMR (Electronic Medical Records) or EHR systems for documentation purposes. Knowledge of medical terminology, medication administration protocols, and basic life support (BLS) including CPR certification. Experience working with individuals with disabilities or developmental disabilities; experience with geriatrics or dementia care is highly valued. Ability to lift heavy objects safely and assist with patient transfers using appropriate techniques such as Hoyer lifts or other transfer methods. Strong communication skills for effective client interaction and collaboration with healthcare teams.
