Duties Assist patients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and toileting to promote comfort and dignity Monitor patient health by observing vital signs, reporting changes, and maintaining accurate medical documentation using Electronic Health Record (EHR) systems Support medication administration following prescribed protocols while ensuring compliance with home healthcare regulations and HIPAA confidentiality standards Provide specialized care including Alzheimer's care, dementia care, hospice support, and end-of-life assistance with sensitivity and professionalism Aid with mobility and patient transfer methods utilizing equipment such as Hoyer lifts or patient transfer techniques to ensure safety for both client and caregiver Perform light housekeeping tasks like cleaning, laundry, meal preparation, and maintaining a safe environment to promote overall well-being Communicate effectively with clients and family members to ensure personalized care plans are followed and needs are met promptly Requirements Home Health aide certification Proven experience working with individuals with disabilities, developmental delays, or geriatric patients in home or long-term care settings Knowledge of medical terminology, catheter care, wound management, and basic life support (BLS) including CPR certification Familiarity with EMR/EHR systems for accurate documentation of patient care activities Ability to lift heavy objects safely and perform physical tasks required for patient transfer and mobility assistance Strong understanding of home health care regulations, patient confidentiality (HIPAA), and client communication best practices Prior experience in assisted living facilities, nursing homes, or home health agencies is highly desirable Valid certification in first aid and CPR; additional certifications such as hospice or dementia care are a plus Join us in delivering compassionate home health care that empowers clients to live safely and comfortably in their own homes.
