Job Summary Join our compassionate healthcare team as a Home Health Aide (HHA), where your dedication and caregiving skills will make a meaningful difference in the lives of patients receiving home and community-based care. In this vital role, you will provide personalized assistance to individuals with diverse needs, including seniors, those with disabilities, and patients requiring specialized care such as Alzheimer's or dementia support. This position offers an opportunity to work closely with healthcare professionals and contribute to improving patient well-being through dedicated caregiving. Duties Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support using equipment like Hoyer lifts or wheelchairs. Monitor patient health by observing vital signs such as blood pressure, pulse, temperature, and respiration; document findings accurately using Electronic Health Record (EHR) systems while maintaining HIPAA compliance. Support medication administration under supervision, ensuring proper timing and dosage while adhering to medical documentation standards. Provide specialized care for patients with dementia or Alzheimer's disease, including memory care techniques and ensuring a safe environment. Perform household tasks such as meal preparation, light cleaning, laundry, and maintaining a hygienic living space to promote overall health and comfort. Assist with medical procedures like catheter care or hospice support as directed by healthcare providers. Use medical terminology appropriately when documenting patient interactions and updating medical records; communicate effectively with nurses and other healthcare team members. Employ patient observation skills to detect changes in condition or behavior that may require escalation or intervention. Support patients during activities outside the home or in community settings, fostering independence and engagement. Maintain confidentiality and adhere strictly to HIPAA regulations to protect patient privacy at all times. Experience Prior experience working in assisted living facilities, nursing homes, or home health environments is highly preferred. Demonstrated ability to assist with ADLs and provide compassionate senior care or disability support. Familiarity with EMR (Electronic Medical Records) or EHR systems for medical documentation is advantageous. Experience caring for individuals with developmental disabilities or memory impairments such as Alzheimer's disease enhances your candidacy. Knowledge of medical terminology, CPR certification, and skills in catheter care are desirable assets. Long-term care experience combined with home & community care background will set you apart. Proven ability to work effectively with diverse populations including hospice patients or those requiring hospice care support.
