As a Home Health Aide, you will be the trusted companion and caregiver, ensuring comfort, safety, and dignity for your clients while adhering to all healthcare regulations and standards. Responsibilities Assist patients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, toileting, and mobility to promote independence and well-being Monitor patient health through observation of vital signs, patient condition, and behavioral changes; report findings accurately using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems Support medication administration following prescribed protocols while maintaining strict adherence to HIPAA regulations and patient confidentiality Provide specialized care including Alzheimer's care, dementia care, hospice support, and end-of-life assistance with compassion and professionalism Perform catheter care, wound dressing assistance, light housekeeping tasks like cleaning and laundry, meal preparation, and ensuring a safe environment for clients Utilize patient transfer methods such as Hoyer lifts or other mobility aids safely to prevent injury for both patient and caregiver Document all patient interactions meticulously in medical records and communicate effectively with healthcare teams regarding patient needs or changes Experience Prior experience working in assisted living facilities, nursing homes, or home healthcare agencies is highly preferred Demonstrated knowledge of medical terminology, home health regulations, and caregiving best practices for geriatric patients and individuals with disabilities Hands-on experience with EMR/EHR systems for accurate documentation of patient care activities Training or certification in CPR, First Aid, Basic Life Support (BLS), or related emergency response skills is required Proven ability to handle heavy lifting safely and perform physically demanding tasks such as assisting with transfers or mobility support Experience working with individuals with developmental disabilities or memory impairments enhances your ability to provide tailored care in diverse settings Strong communication skills to effectively engage with clients and their families while maintaining professionalism and empathy Join us in delivering exceptional home health care that empowers individuals to live comfortably and independently.
