Duties Assist individuals with Activities of Daily Living (ADLs), including personal hygiene, dressing, grooming, and mobility support using manual handling techniques and patient transfer methods such as Hoyer lifts. Monitor patients' vital signs and observe for any changes in health status, documenting findings accurately while adhering to HIPAA regulations to ensure confidentiality. Administer medications as prescribed, following care plans and medication administration protocols to ensure safety and compliance. Prepare nutritious meals tailored to dietary needs and assist with meal planning and preparation to promote healthy living. Support residents with community integration by providing transportation assistance for appointments, outings, or errands. Maintain a clean and safe environment by performing light cleaning duties and ensuring infection control standards are met. Document care activities meticulously, communicate effectively with clients and team members, and participate in ongoing training related to dementia care, Alzheimer's care, autism support, and neurodevelopmental disorders. Skills Proven experience working with individuals with disabilities or developmental disabilities in settings such as group homes or assisted living facilities. Strong patient observation skills to detect changes in condition or behavior promptly. Knowledge of patient monitoring techniques including vital signs measurement and dementia or memory care strategies. Ability to perform patient transfers safely using equipment like Hoyer lifts and adhere to manual handling safety standards. Familiarity with HIPAA regulations to protect client privacy at all times. Excellent communication skills for engaging with clients, families, and healthcare teams effectively. Basic first aid and CPR certification to respond confidently during emergencies. Experience working with individuals with neurodevelopmental disorders such as autism or intellectual disabilities support needs. Capable of heavy lifting and physical tasks required for patient transfer and caregiving duties. Home health or hospice care experience is a plus for understanding comprehensive patient needs across different environments.
