Duties Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using patient transfer methods and equipment such as Hoyer lifts Monitor and observe patients' conditions, recording vital signs and medical observations accurately in Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems Administer medications following medical documentation protocols while maintaining strict adherence to home health care regulations and HIPAA confidentiality standards Provide specialized care for patients with dementia, Alzheimer's disease, or other cognitive impairments, including memory care and behavioral support strategies Support patients with catheter care, wound dressing changes, and end-of-life care with compassion and professionalism Assist with meal preparation, light housekeeping, laundry, and cleaning to ensure a safe and comfortable living environment Communicate effectively with clients, families, and healthcare teams to coordinate care plans and update medical records regularly Experience Prior experience working in assisted living facilities, nursing homes, or home healthcare agencies is highly preferred Demonstrated knowledge of medical terminology, patient transfer techniques, and vital signs monitoring Experience caring for geriatric patients or individuals with disabilities, including developmental disabilities or dementia Familiarity with home health regulations, hospice care practices, and HIPAA compliance standards Certification in First Aid and Basic Life Support (BLS), CPR is required; additional certifications such as hospice care or heavy lifting training are advantageous Proven ability to work independently in live-in employment settings or community-based environments Strong communication skills for client interaction and documentation accuracy Embark on a fulfilling career where your caregiving skills directly enhance the quality of life for those you serve.
