Job Description
Job Summary Join our compassionate team as a Home Health Aide, providing vital support to individuals in their homes to promote independence, comfort, and well-being. This role offers a rewarding opportunity to make a meaningful difference in the lives of patients by delivering personalized care tailored to their unique needs. Working Monday through Friday from 5 pm to 9 pm, you will be an essential part of a dedicated caregiving team committed to enhancing quality of life through attentive and respectful assistance. Responsibilities Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support Observe and monitor patient health status, recording vital signs and noting any changes or concerns Remind clients to take their medications. Provide specialized care for individuals with dementia, Alzheimer's disease, or other memory impairments, including memory care techniques Operate Electronic Health Record (EHR) and Electronic Medical Record (EMR) systems accurately to document patient interactions and medical information Assist with catheter care, wound care, and other medical procedures as trained and authorized Support hospice patients with compassionate end-of-life care in accordance with care plans Assist with feeding while respecting dietary restrictions and preferences Perform light housekeeping tasks including cleaning, laundry, and maintaining a safe environment for patients Collaborate effectively with healthcare teams, ensuring compliance with HIPAA regulations and medical documentation standards Engage in ongoing training related to patient observation, CPR certification, medical terminology, and home health best practices Qualifications Prior experience in assisted living facilities, nursing homes, or home health environments preferred Demonstrated ability in patient observation and working with individuals with disabilities or developmental challenges Knowledge of medical terminology, medication administration, and vital signs monitoring Certification in CPR preferred; additional certifications such as hospice care or dementia training are advantageous Experience operating EMR/EHR systems for accurate documentation Familiarity with long-term care practices including Alzheimer's and memory care techniques Ability to work independently while maintaining excellent communication within a caregiving team Compassionate attitude with strong attention to detail and respect for patient privacy under HIPAA guidelines Physical ability to assist with mobility and perform light housekeeping tasks Embark on a fulfilling career where your dedication directly improves lives.
