Responsibilities Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support Monitor patient health by observing vital signs and reporting changes to healthcare professionals Administer medications and perform catheter care following medical instructions and home healthcare regulations Support patients with memory care needs such as Alzheimer's disease and dementia, ensuring safety and comfort Document patient care activities accurately using Electronic Health Record (EHR) systems and maintain confidentiality in accordance with HIPAA standards Assist with meal preparation, light housekeeping, laundry, and maintaining a clean, safe environment Use patient transfer methods including Hoyer lifts and proper patient transfer techniques to ensure safety during mobility assistance Skills Proven experience working with individuals with disabilities, developmental disabilities, or geriatrics in home or long-term care settings Strong knowledge of medical terminology, medication administration, vital signs collection, and basic life support (BLS) including CPR and first aid certification Familiarity with EMR/EHR systems for medical documentation and home health agency procedures Ability to perform heavy lifting safely and operate equipment such as Hoyer lifts for patient transfers Excellent communication skills for client interaction, care planning, and team collaboration Knowledge of home health care regulations, hospice care principles, and end-of-life support practices Experience working with geriatric patients in assisted living or nursing home environments is highly desirable Join us in delivering compassionate home health care that enriches lives!
