Job Description

Job Overview We are seeking a compassionate and dedicated Certified Home Health Aide to provide exceptional in-home care to clients across diverse settings. As a vital member of our healthcare team, you will deliver personalized assistance that promotes independence, comfort, and well-being. Your energetic approach and attention to detail will ensure each patient receives the highest quality of care, fostering trust and positive outcomes. This role offers an opportunity to make a meaningful difference in the lives of individuals requiring support with daily activities, medical needs, and emotional comfort. Duties Assist patients with activities of daily living (ADLs), including bathing, dressing, grooming, and mobility support using patient transfer methods and equipment such as Hoyer lifts. Monitor and document vital signs, medication administration, and changes in patient condition using Electronic Health Record (EHR) systems while maintaining strict adherence to HIPAA regulations to protect patient confidentiality. Perform light housekeeping duties such as cleaning, laundry, meal preparation, and ensuring a safe environment aligned with home healthcare regulations. Communicate effectively with clients and their families to ensure understanding of care plans and address any concerns promptly. Assist with patient transfer methods using proper techniques or equipment like Hoyer lifts or gait belts to ensure safety during mobility assistance. Skills Proven experience working with geriatric patients, individuals with disabilities, or those requiring hospice or long-term care. Strong knowledge of medical terminology, medication administration protocols, and vital signs monitoring. Skilled in patient observation to detect changes in health status or behavioral patterns promptly. Familiarity with EMR/EHR systems for accurate medical documentation and record keeping. Excellent communication skills for client interaction and team collaboration. Ability to perform heavy lifting safely and manage physical demands associated with caregiving tasks. Certifications in CPR and First Aid; additional training in Alzheimer's/dementia care is preferred. Knowledge of home health care assistance standards, home & community care practices, and working within home health agency guidelines. $15.00 - $16.00 per hour
