Overview Evergreen Healthcare Solutions is seeking a compassionate, reliable, and safety-focused Direct Support Professional (DSP) to provide individualized support to an individual with developmental and/or health-related needs. Responsibilities Assist with wheelchair-to-vehicle and vehicle-to-wheelchair transfers using proper safety techniques. Safely secure and handle the individual's wheelchair during transportation. Follow established mobility, transfer, and person-centered care plans. Assist with ADLs, including bathing, dressing, grooming, toileting, feeding, and mobility. Use approved transfer equipment, including Hoyer lifts, as trained and required. Monitor and report changes in the individual's health, behavior, or overall condition. Provide medication support as authorized and in accordance with the care plan and applicable regulations. Prepare meals according to dietary needs and maintain a clean and safe living environment. Support community outings, appointments, recreational activities, and social engagement. Complete required documentation and maintain HIPAA confidentiality. Follow agency policies, DDA requirements, and all health and safety procedures. Qualifications Previous DSP, caregiver, home health, group home, assisted living, or related experience preferred. Experience supporting individuals with developmental disabilities or complex care needs is a plus. Ability to safely perform transfers and assist with mobility equipment. Valid driver's license and reliable transportation required. Must be authorized to work in the U.S. and successfully complete required background checks. Strong communication, patience, reliability, and a compassionate attitude. CPR/First Aid, CMT, DDA, Bloodborne Pathogens, and Mandt certifications are highly desirable.
