Duties Assist residents with Activities of Daily Living (ADLs), including personal hygiene, dressing, grooming, and mobility support using manual handling techniques such as Hoyer lifts and patient transfer methods. Monitor residents' health by taking vital signs, observing patient conditions, and reporting any changes or concerns promptly. Administer medications following care plans and ensure compliance with HIPAA regulations to maintain confidentiality. Prepare nutritious meals, assist with meal planning, and ensure proper food safety standards are upheld. Support residents during community outings or appointments by providing personal transportation assistance and ensuring safety throughout activities. Maintain a clean and safe environment by performing light cleaning duties and adhering to infection control protocols. Document care activities accurately in resident records, including patient monitoring notes, medication administration logs, and incident reports. Qualifications Prior experience working in group homes, assisted living facilities, nursing homes, or home health environments is highly preferred. Knowledge of dementia care, Alzheimer's care, memory care, autism support, intellectual disabilities support, neurodevelopmental disorders, and developmental disabilities is advantageous. Demonstrated ability to observe patients carefully and respond appropriately to their needs with patience and empathy. Certification in CPR and First Aid is required; additional certifications such as HIPAA training or medication administration are a plus. Experience operating patient transfer equipment like Hoyer lifts or patient personal care assistance tools is desirable. Strong communication skills for effective client interaction and team collaboration. Ability to perform heavy lifting safely and handle manual tasks required for patient transfers or mobility assistance. Knowledge of home & community care standards along with experience working with seniors or individuals with disabilities is beneficial.
