Overview Join our compassionate team as a Caregiver / Home Health Aide and build your schedule while making a meaningful difference in people's lives! In this vital role, you will provide personalized care to individuals across various settings, including home care, assisted living, nursing homes, and memory care facilities. Responsibilities Deliver direct patient support by assisting with activities of daily living (ADLs), including bathing, dressing, grooming, and mobility assistance using proper patient transfer methods such as Hoyer lifts and manual handling techniques. Monitor patients' vital signs and observe for changes in health status, documenting findings accurately in care plans while adhering to HIPAA regulations to protect client confidentiality. Prepare nutritious meals tailored to individual dietary needs and assist with meal consumption when necessary. Support clients with medication administration following prescribed instructions and ensure compliance with medication schedules. Maintain a clean and safe environment by performing light housekeeping tasks such as cleaning client spaces and ensuring safety protocols are followed at all times. Provide transportation assistance for medical appointments, errands, or community activities using personal transportation or agency-provided resources. Communicate effectively with clients and their families about care routines, progress updates, and resident rights to foster trust and transparency. Experience Prior experience working in group homes, assisted living facilities, nursing homes, or home health environments is highly preferred. Demonstrated ability to observe patients carefully and respond promptly to changes in condition or behavior. Familiarity with dementia care, Alzheimer's care, memory care, hospice care, and supporting individuals with neurodevelopmental or developmental disabilities enhances your effectiveness in this role. Knowledge of HIPAA compliance, patient monitoring techniques, medication administration procedures, and first aid/CPR certification is essential. Experience working with individuals with autism or intellectual disabilities supports personalized caregiving approaches. Ability to perform heavy lifting safely when assisting with patient transfers or mobility support. Strong communication skills for engaging with clients respectfully while maintaining their rights and dignity at all times.
