Applicants should:

Be at least 18 years old Have a valid government-issued ID Be dependable and professional Have reliable transportation to travel to clients' homes throughout their accepted service area Have a working smartphone for EVV Comfortable providing personal care Able to follow written care instructions Willing to complete required background screenings Willing to complete required agency and EVV training Willing to complete applicable Michigan Medicaid/CHAMPS enrollment requirements Previous caregiving experience is preferred. Duties Provide personal care assistance including bathing, grooming, dressing, and mobility support to patients of all ages. Assist with Activities of Daily Living (ADLs) such as toileting, feeding, and transferring using patient transfer methods and equipment like Hoyer lifts. Support patients with memory care needs such as Alzheimer's disease and dementia by implementing specialized care plans and maintaining a safe environment. Assist with light housekeeping tasks including cleaning, laundry, meal preparation, and ensuring a clean, safe living space. Maintain patient confidentiality adhering to HIPAA regulations while documenting medical information and communicating effectively with clients and families. Important Information Two Hearts Community Living is currently recruiting caregivers for upcoming client assignments throughout Wayne, Monroe, and Macomb County.
