Duties Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using patient transfer methods such as Hoyer lifts when necessary Monitor patient observations by regularly checking vital signs and reporting any changes promptly Administer medications following medical documentation protocols and home healthcare regulations Support clients with meal preparation, light housekeeping, laundry, and cleaning to maintain a safe and comfortable environment Document medical records accurately using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems while maintaining patient confidentiality in compliance with HIPAA standards Provide specialized care for clients with dementia or Alzheimer's disease, including memory care techniques and behavioral management strategies Assist with end-of-life care and hospice support, ensuring comfort and emotional well-being for patients and their families Qualifications Prior experience working in assisted living facilities, nursing homes, or home healthcare agencies is highly preferred Knowledge of medical terminology, catheter care, and basic life support (BLS) or CPR certification is required Experience working with individuals with developmental disabilities or disabilities is advantageous Strong patient observation skills and the ability to communicate effectively with clients and family members Ability to perform heavy lifting safely and use patient transfer equipment such as Hoyer lifts when needed Familiarity with home health regulations, medication administration procedures, and documentation standards Compassionate attitude with a commitment to providing high-quality patient care in a home & community setting Join us to be part of a caring team that values your expertise!
