Location:

Individual supportive living (ISL) home located in Maryland Heights Schedule The ISL shifts are 8-hour shifts - 7:00am to 3:00pm, 3:00pm to 11:00pm, 11:00pm to 7:00am. The PRN DSP will provide coverage as shifts are available.

Key Responsibilities:

Provide direct support to individuals with intellectual and developmental disabilities in accordance with the individual's Person-Centered Support Plan (PCSP) and agency policies. Assist individuals with activities of daily living (ADLs), including personal hygiene, grooming, bathing, dressing, toileting, meal preparation, medication support, and housekeeping. Promote independence by teaching, modeling, and reinforcing skills related to self-care, communication, socialization, community integration, and daily living. Support individuals in accessing and participating in community activities, appointments, employment, volunteer opportunities, recreational activities, and social events. Implement individualized behavioral support strategies and interventions as outlined in approved behavior support plans. Monitor for and report changes in health, behavior, emotional well-being, or environmental concerns to supervisors and appropriate team members. Maintain a safe, clean, and healthy living environment while ensuring the individual's rights, dignity, privacy, and personal preferences are respected. Complete required documentation accurately and timely, including progress notes, medication administration records, incident reports, and other required records. Assist with transportation of individuals to appointments, community activities, employment sites, and other approved destinations. Respond appropriately to emergencies, incidents, and crisis situations according to agency policies and training requirements. Advocate for individuals' rights, choices, and personal goals while encouraging self-determination and informed decision-making. Follow all Medicaid, Department of Mental Health (DMH), Division of Developmental Disabilities (DDD), and agency regulations, policies, and procedures. Participate in required trainings, staff meetings, and ongoing professional development activities. Maintain professional and positive relationships with individuals served, family members, guardians, team members, and community partners. Perform other duties as assigned to support the health, safety, and quality of life of individuals receiving services. Qualifications and Requirements

• Must be at least 18 years of age.
• High school diploma or GED required.
• Valid driver's license, reliable transportation, and proof of automobile insurance required.
• Ability to pass criminal background checks, Family Care Safety Registry screening, and any other screenings required by state regulations.
• Ability to obtain and maintain certification in CPR, First Aid, Medication Administration, and other required trainings.
• Ability to complete all required agency orientation and ongoing training requirements.
• Ability to maintain confidentiality and comply with HIPAA and agency confidentiality policies.
• Strong communication and interpersonal skills.
• Ability to work independently and exercise sound judgment in supporting individuals in community and home settings.
• Ability to accurately complete documentation and maintain records.
• Ability to safely assist individuals with mobility, transfers, and personal care needs as required by the individual's support plan.
• Ability to lift, push, pull, and carry up to 50 pounds and perform physical tasks associated with supporting individuals.
• Ability to work flexible schedules, including evenings, weekends, holidays, and overnight shifts as needed.
• Experience supporting individuals with intellectual and developmental disabilities preferred but not required.
• Demonstrated commitment to treating individuals with dignity, respect, and person-centered values.
