Job Overview We are seeking a compassionate and dedicated Personal Care Assistant/Certified Nursing Assistant (CNA) to provide exceptional support and care to our clients in various settings, including home healthcare, assisted living, and long-term care facilities. Responsibilities Assist clients with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, toileting, and mobility support using patient transfer methods and equipment like Hoyer lifts when necessary. Monitor patient health by observing vital signs including blood pressure, pulse, temperature, and respiration; document findings accurately using Electronic Health Record (EHR) systems while maintaining patient confidentiality in compliance with HIPAA regulations. Support medication administration under supervision or as permitted by state law; ensure proper handling and documentation of medical records. Provide specialized care for individuals with dementia or Alzheimer's disease, including memory care techniques and end-of-life care when applicable. Assist with catheter care, wound management, and other medical procedures following home healthcare regulations and best practices. Maintain a clean and safe environment by performing light housekeeping tasks such as laundry, cleaning, meal preparation, and ensuring infection control standards are met. Communicate effectively with clients, family members, and healthcare teams to ensure personalized care plans are followed; utilize patient personal care assistance skills to foster trust and comfort. Qualifications Valid CNA certification or equivalent licensure required; prior experience in assisted living or nursing home settings is highly preferred. Demonstrated experience working with individuals with developmental disabilities or geriatrics; familiarity with dementia care and hospice settings is a plus. Knowledge of medical terminology, home health care assistance protocols, and medication management. Ability to perform heavy lifting safely and operate patient transfer equipment such as Hoyer lifts; physical stamina to support active caregiving duties. Proficiency in EMR/EHR systems for accurate medical documentation; understanding of home health care regulations including HIPAA compliance. Certification in First Aid and Basic Life Support (BLS), CPR required; additional certifications in hospice or end-of-life care are advantageous. Excellent communication skills to effectively engage with patients, families, and healthcare professionals.
