Responsibilities Assist clients with activities of daily living (ADLs), including bathing, dressing, grooming, and toileting. Monitor patients' health status by taking vital signs and observing for any changes or concerns. Administer medications accurately following care plans and HIPAA regulations to ensure patient safety and confidentiality. Support mobility using equipment such as Hoyer lifts or assisting with transfers to prevent injury. Prepare nutritious meals tailored to individual dietary needs and assist with feeding when necessary. Maintain a clean and safe environment by performing light housekeeping tasks like cleaning and laundry. Document patient progress, update care plans, and communicate effectively with healthcare teams regarding patient needs or changes. Skills Proven experience in home & community care, assisted living, nursing homes, or long-term care environments. Knowledge of patient observation techniques and ability to monitor vital signs accurately. Certification in CPR and First Aid; experience with Alzheimer's care, dementia care, memory care or hospice is highly valued. Familiarity with working with individuals with developmental disabilities or autism spectrum disorder. Ability to lift heavy objects safely and use assistive devices such as Hoyer lifts or resident's for transfers. Strong understanding of HIPAA regulations to protect patient privacy and confidentiality.
