Job Description

Job Overview Join our compassionate healthcare team as a Home Health Aide, where your dedication and caregiving skills will make a meaningful difference in the lives of patients receiving home-based medical and personal care. In this vital role, you will provide personalized assistance to individuals with diverse needs, promoting independence, comfort, and well-being in their own homes. Your energetic approach and attention to detail will help ensure that each patient receives the highest quality care in accordance with home healthcare regulations and best practices. This paid position offers an opportunity to grow your caregiving expertise while making a positive impact in the community. Duties Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using patient transfer methods and equipment such as Hoyer lifts when necessary Monitor and document patient health status by taking vital signs, recording medical information using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems, and reporting changes to supervising nurses or physicians Support medication administration following prescribed protocols while maintaining strict adherence to HIPAA guidelines and patient confidentiality standards Provide specialized care for patients with dementia, Alzheimer's disease, or other cognitive impairments, including memory care techniques and behavioral management strategies Assist with catheter care, wound dressing changes, end-of-life care, and other complex medical procedures under supervision or according to training Prepare nutritious meals tailored to dietary restrictions and assist with feeding when necessary; perform light cleaning and laundry to maintain a safe and hygienic environment Document all care activities accurately in medical records and communicate effectively with clients, family members, and healthcare teams to ensure seamless continuity of care Skills Proven experience working with individuals with disabilities, developmental delays, or geriatric patients in home healthcare or long-term care settings Strong knowledge of medical terminology, home health care regulations, and documentation standards such as HIPAA compliance Proficiency in using EMR/EHR systems for accurate recordkeeping and patient data management Excellent patient observation skills to identify changes in condition or behavior promptly Ability to perform physical tasks including heavy lifting, patient transfers using Hoyer lifts or other equipment, and supporting mobility needs safely Certifications such as First Aid, Basic Life Support (BLS), CPR, and hospice or end-of-life care training are highly valued Effective communication skills for client interaction, team collaboration, and maintaining patient confidentiality at all times Join us in delivering compassionate home health care that empowers patients to live safely and comfortably. Pay:
$18.00 - $20.00 per hour
