Live-in Home Health Aide - Week on/Week off Join our team as a Live-in Care Aide in Germantown, NY. We are seeking an experienced Care Aide with a valid HHA or CNA certification. This week on/week off live-in role pays per diem and includes room and board. Candidates must have a valid driver's license. As a live-in caregiver, you will be an essential part of promoting independence while delivering high-quality home healthcare services. Responsibilities Provide personal care assistance with bathing, dressing, grooming, and mobility support . Assist with/reminders for Activities of Daily Living (ADLs) such as toileting, feeding, and medication while maintaining patient dignity and confidentiality in accordance with HIPAA regulations. Monitor patient health by observing and reporting any changes promptly. Support clients with dementia or cognitive impairment by implementing memory care strategies and ensuring a safe environment. Perform light housekeeping duties including cleaning, laundry, meal preparation tailored to dietary needs, and maintaining a hygienic living space. Document all patient interactions and medical records using Electronic Health Record (EHR) systems or EMR systems to ensure precise medical documentation. Assist with end-of-life or hospice care with compassion and sensitivity while respecting patient wishes and comfort. Support client communication by engaging empathetically and providing emotional support during difficult times. Use proper patient transfer methods safely when assisting with mobility or repositioning patients to prevent falls or injuries. Skills Proven experience working with geriatric patients, individuals with disabilities, or those requiring long-term care in home or assisted living settings. Strong knowledge of medical terminology, medication administration procedures, and vital signs monitoring. Familiarity with home health care assistance including catheter care, hospice care, dementia care, and Alzheimer's support strategies. Ability to operate EMR/EHR systems efficiently for accurate medical records management. Excellent patient observation skills coupled with compassionate caregiving abilities; able to respond swiftly to changing health conditions. Knowledge of home health regulations, HIPAA compliance, and maintaining patient confidentiality at all times. Physical stamina for heavy lifting and performing tasks such as laundry or cleaning; experience working with Hoyer lifts preferred. Effective client communication skills; capable of building trusting relationships through active listening and empathy. Certification in First Aid and Basic Life Support (BLS), including CPR training; additional certifications in medical documentation or hospice care are advantageous.
