Job Summary Join our compassionate team as a Home Health Aide & Caregiver, where your dedication will make a meaningful difference in the lives of individuals requiring personalized care. In this vital role, you will provide comprehensive support to clients in their homes or assisted living facilities, promoting independence, comfort, and well-being. Your energetic approach and attention to detail will ensure each person receives attentive, respectful, and high-quality care tailored to their unique needs. This position offers an opportunity to work closely with diverse populations, including seniors, individuals with disabilities, and those experiencing memory loss or chronic health conditions. Responsibilities Assist clients with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts when necessary Monitor and observe patients' health status by recording vital signs and noting changes in condition or behavior Support medication administration following prescribed guidelines while maintaining accurate medical documentation and adhering to HIPAA regulations. Prepare nutritious meals and assist with feeding while accommodating dietary restrictions and preferences Maintain a clean and safe environment by performing light housekeeping tasks such as laundry and general cleaning in compliance with health standards Document patient information accurately using Electronic Health Record (EHR) systems or Electronic Medical Records (EMR systems) to ensure seamless communication among care teams Support clients' emotional well-being through compassionate interaction and engagement in meaningful activities Collaborate effectively with healthcare providers to ensure comprehensive patient care in home or community settings Requirements Proven experience as a Home Health Aide, Caregiver, or in a similar long-term care setting such as nursing homes or assisted living facilities. Certification in CPR (Cardiopulmonary Resuscitation) is required; additional certifications such as HIPAA training are preferred. Ability to work independently while following detailed care plans and instructions from healthcare professionals Experience working with individuals with developmental disabilities or dementia is highly desirable Familiarity with EMR/EHR systems for medical documentation and record keeping Strong observational skills for patient monitoring and reporting changes promptly Physical ability to assist with mobility support tasks including the use of lifts like Hoyer lifts and performing transfers safely Compassionate attitude combined with excellent communication skills to foster trust and rapport with clients and their families Join us in providing exceptional home health care that enriches lives through dedicated service. $14.00 - $18.00 per hour
