Job Summary We are seeking a compassionate and dedicated Home Health Aide to provide exceptional in-home care to clients across diverse needs. In this vital role, you will support individuals with daily activities, medical needs, and emotional well-being, fostering independence and enhancing quality of life. Responsibilities Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using tools such as Hoyer lifts or other assistive devices. Monitor and record vital signs such as blood pressure, pulse, temperature, and respiration, ensuring accurate medical documentation within Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems. Provide medication administration following prescribed schedules and protocols while maintaining strict adherence to HIPAA regulations for patient privacy. Support clients with specialized care needs such as dementia, Alzheimer's disease, memory care, hospice care, or individuals with developmental disabilities. Perform light housekeeping tasks including cleaning, laundry, meal preparation, and ensuring a safe living environment for clients. Document all patient interactions thoroughly in medical records and communicate effectively with healthcare teams regarding patient status or changes in condition. Use medical terminology accurately when recording observations or reporting concerns to ensure clear communication among care providers. Skills Proven experience in home health care or long-term care settings with familiarity working with seniors and individuals with disabilities. Strong patient observation skills to monitor health status and recognize signs requiring medical attention. Knowledge of assisting with ADLs, medication administration, catheter care, and hospice or dementia-specific needs. Experience working with EMR/EHR systems for efficient documentation and record keeping. Ability to operate medical equipment such as Hoyer lifts or vital signs monitors confidently and safely. Excellent communication skills combined with compassionate caregiving to foster trust and rapport with clients and their families. Certifications including CPR/First Aid are required; additional training in Alzheimer's care or hospice support is a plus.
