Job Summary We are seeking a dedicated and compassionate Certified Home Health Aide to deliver exceptional in-home care to our clients. The ideal candidate will possess a strong commitment to patient well-being, demonstrate excellent communication skills, and have experience working with diverse populations, including geriatric patients and individuals with disabilities. This role offers an opportunity to make a meaningful difference in the lives of those we serve by providing essential personal care and health support within their homes. Responsibilities Assist patients with activities of daily living (ADLs), including bathing, dressing, grooming, and mobility assistance. Monitor and observe patient health status, documenting vital signs and changes accurately in electronic health record (EHR) or electronic medical record (EMR) systems. Administer medications and perform catheter care following medical instructions and home healthcare regulations. Support patients with memory care needs such as Alzheimer's disease and dementia, ensuring a safe environment. Assist with light housekeeping tasks such as cleaning, laundry, and meal preparation to promote overall well-being. Utilize patient transfer methods and equipment like Hoyer lifts to ensure safe mobility assistance. Maintain patient confidentiality in accordance with HIPAA regulations while communicating effectively with clients and healthcare teams. Skills Proven experience working with individuals with developmental disabilities, geriatrics, or in hospice care settings. Strong knowledge of medical terminology, vital signs measurement, medication administration, and basic life support (BLS) or CPR certification. Familiarity with EMR/EHR systems for accurate documentation of patient care activities. Ability to perform heavy lifting safely and operate patient transfer devices such as Hoyer lifts. Excellent caregiving skills with a focus on patient personal care assistance and client communication. Knowledge of home health care assistance standards, including end-of-life care and dementia-specific strategies. Experience working within home health agency protocols and understanding of home health regulations. Pay & Benefits $18.00 per visit 58.5 cents per mile Full benefit package available when aide reaches full-time status including health and dental insurance, vision insurance, paid time off, and life insurance.
