Job Description

Job Overview Join our compassionate team as a Home Health Aide/Caregiver and make a meaningful difference in the lives of individuals requiring personalized care and support. In this vital role, you will provide essential assistance to clients in their homes, promoting independence, safety, and well-being. Your energetic and caring approach will help clients maintain their dignity while receiving high-quality, respectful care tailored to their unique needs. This position offers an opportunity to work closely with diverse populations, including seniors, individuals with disabilities, and those receiving hospice or end-of-life care. Responsibilities Deliver personal care assistance including bathing, dressing, grooming, and mobility support to promote client comfort and dignity. Assist with Activities of Daily Living (ADLs) such as toileting, feeding, transferring using patient transfer methods and Hoyer lifts when necessary. Monitor patient health by observing vital signs, recognizing changes in condition, and reporting findings accurately using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems. Support medication administration following medical terminology guidelines and home healthcare regulations while maintaining strict patient confidentiality in compliance with HIPAA standards. Provide companionship and emotional support to clients, fostering a positive environment that encourages mental well-being. Assist with light housekeeping tasks including cleaning, laundry, meal preparation, and ensuring a safe living environment. Document all care activities thoroughly in medical records and communicate effectively with healthcare teams regarding patient status or concerns. Qualifications Proven experience working with individuals with developmental disabilities, dementia or Alzheimer's care, geriatrics, or hospice care. Knowledge of medical terminology, catheter care, medication management, and vital signs monitoring. Familiarity with EMR/EHR systems and home health documentation procedures. Certification in First Aid and Basic Life Support (BLS), including CPR training. Ability to perform heavy lifting safely and operate patient transfer equipment such as Hoyer lifts. Experience working within home healthcare regulations and maintaining patient confidentiality under HIPAA guidelines. Strong communication skills for effective client interaction and team collaboration. Prior experience in assisted living facilities or nursing homes is highly desirable.
