Job Summary We are seeking a compassionate and dedicated Personal Care Assistant to provide essential support and care to individuals in various settings, including assisted living facilities, nursing homes, and community-based environments. Responsibilities Assist residents with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and mobility assistance using equipment like Hoyer lifts or manual handling techniques. Monitor residents' vital signs, observe changes in health status, and report findings according to care plans while maintaining HIPAA compliance. Prepare nutritious meals tailored to individual dietary needs and assist with meal consumption to promote proper nutrition. Support medication administration under supervision or as trained, ensuring accuracy and adherence to prescribed schedules. Maintain a clean and safe environment through regular cleaning duties, laundry support, and infection control practices. Observe residents' behavior and condition, especially those with dementia or neurodevelopmental disorders like autism or intellectual disabilities, documenting any concerns or changes. Follow care plans meticulously, participate in team meetings, and collaborate with healthcare professionals to deliver comprehensive patient-centered support. Experience Prior experience working in group homes, assisted living facilities, nursing homes, or home health environments is highly preferred. Demonstrated ability to work with individuals with developmental disabilities, Alzheimer's disease, dementia, or other memory-related conditions. Knowledge of patient observation techniques, medication administration procedures, and vital signs monitoring. Familiarity with HIPAA regulations and confidentiality standards related to patient information. Experience in caregiving roles involving manual handling equipment such as Hoyer lifts or assisting with ADLs. Strong background in geriatrics, hospice care, social work support, or long-term care settings is advantageous.
