Job Description

Job Overview We are seeking compassionate and dedicated Home Health Aides to provide exceptional in-home care to clients across various settings. This role involves supporting individuals with diverse needs, including seniors, those with disabilities, and patients requiring specialized care such as Alzheimer's or dementia. The ideal candidate will have a strong commitment to patient well-being, excellent communication skills, and experience working in home and community care environments. This position offers an opportunity to make a meaningful difference in clients' lives while working in a supportive and professional setting. Duties Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support Monitor and observe patients' conditions, noting any changes or concerns Administer medications following prescribed schedules and protocols Perform vital signs assessments including blood pressure, pulse, and temperature Support with medical procedures such as catheter care and Hoyer lift transfers Maintain accurate medical documentation using EMR/EHR systems while ensuring compliance with HIPAA regulations Prepare nutritious meals and assist with meal planning tailored to dietary needs Perform light housekeeping tasks including cleaning and laundry to ensure a safe living environment Assist with medical record keeping and documentation of patient care activities Provide emotional support and companionship to promote mental well-being Follow all safety protocols related to hospice care, dementia care, Alzheimer's care, and memory care Experience Candidates should have prior experience in home health or community care settings, including long-term care or assisted living environments. Experience working with individuals with developmental disabilities or cognitive impairments such as dementia or Alzheimer's is highly valued. Familiarity with medical terminology, medication administration, and the use of EMR/EHR systems is preferred. Certifications such as CPR are required. Prior experience working with hospice patients or providing hospice care is advantageous. Candidates should demonstrate strong patient observation skills, compassion, patience, and the ability to work independently while adhering to all safety standards. Experience with medical documentation, caregiving for seniors or individuals with disabilities, and proficiency in activities like cooking and cleaning are also beneficial for success in this role.
