Job Description

Not Forgotten Home & Community Services provides direct ADL support to individuals with developmental disabilities (IDD). We have current opportunities for PT Direct Support Professionals in the North Hills, South Hills, and Plum areas of Pittsburgh for various shifts including: Monday- Friday and Weekends 7am - 3pm, 3pm-11pm, Overnights 11pm-7am Anticipated start date: October 5th/6th
RESPONSIBILITIES
Assist the individual in performing routine personal care tasks, adaptive daily living skills, and assistance in physical transfers and/or ambulation according to the Individual Service Plan (ISP). Perform household tasks such as routine cleaning and laundry, bed making, and monitor nutritional meal preparation as related to the care of the individual. Inform Program/Team Lead in a timely manner matters relating to the care of the individual such as, schedule changes, availability, hospitalization, injuries, environmental issues effecting the health and welfare of the individual. Maintains a healthy and safe environment for each individual. Continually practices universal safety precautions regarding infectious diseases. Maintains a professional and cooperative working relationship with families and other team members. Participate in Circle(s) of Support for personal future planning. Completing documentation of activities at shift change. Ensures shift documentation is complete and accurate. Performs miscellaneous job-related duties as assigned by Program/Team Lead, Residential Coordinator or member of senior management.
KNOWLEDGE, SKILLS, AND ABILITIES
Ability to use independent judgment, critical thinking and professional communication skills to complete daily job duties. Ability to work flexible hours to periodically cover call offs and other emergency situations. Knowledge of philosophy of IDD and current concepts on IDD. Strong interpersonal skills and the ability to develop strong working relationships with care team members.
QUALIFICATIONS
A GED/High School Diploma, associate's degree, or bachelor's degree. Prior experience providing direct ADL support preferred, however, we will train anyone with a passion to care for the IDD community.
WORKING CONDITIONS
May transport participants in own vehicle (if applicable). Work will take place in the community and in the homes.

PHYSICAL REQUIREMENTS
While performing the duties of this job, the employee is occasionally required to stand; walk; sit; use hands to finger, handle, or feel objects, tools or controls; reach with hands and arms; climb stairs; balance; stoop, kneel, crouch or crawl; talk or hear; and taste or smell. The employee must occasionally lift or move up to 50 pounds. Pay:
$17.00 - $23.00 per hour
