In this vital role, you will provide compassionate assistance with daily activities, promote independence, and ensure the safety and well-being of your clients in their home or community settings. This position offers an opportunity to work closely with diverse populations, including seniors, individuals with disabilities, and those receiving hospice or dementia care. Responsibilities Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, toileting, and mobility support using proper patient transfer methods and equipment like Hoyer lifts when necessary. Monitor and observe patients' health status by taking vital signs, documenting medical information accurately using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems, and reporting any changes to healthcare professionals promptly. Support medication management by administering prescribed medications following medical terminology guidelines and home healthcare regulations while maintaining strict patient confidentiality in compliance with HIPAA standards. Provide specialized care for clients with Alzheimer's disease, dementia, or other memory impairments through tailored memory care techniques and compassionate engagement. Perform light housekeeping duties including cleaning, laundry, meal preparation, and ensuring a safe environment that promotes health and comfort. Assist with medical procedures such as catheter care, wound care, or other treatments under supervision or as trained. Maintain detailed medical documentation of patient care activities, observations, and incidents to ensure continuity of care and compliance with regulatory standards. Skills Proven experience working with individuals with developmental disabilities, geriatrics, or in hospice settings is highly valued. Strong knowledge of home health care assistance protocols, home & community care practices, and familiarity with home health agency operations. Excellent patient communication skills combined with the ability to provide personal care assistance empathetically and professionally. Ability to perform patient transfers safely using methods such as patient transfer techniques or Hoyer lifts; capable of heavy lifting when required. Competency in medical terminology, vital signs measurement, medication administration, CPR/First Aid certification, and basic life support skills. Experience working with EMR/EHR systems for accurate documentation; understanding of home health regulations is preferred. Knowledge of dementia care strategies, end-of-life support practices, hospice care procedures, and working effectively with geriatric patients. Ability to maintain patient confidentiality at all times while managing sensitive medical records in compliance with HIPAA standards.
