Job Description

Responsibilities Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using manual handling techniques and transfer methods such as Hoyer lifts. Monitor patients' health by taking vital signs, observing changes in condition, and reporting findings accurately to healthcare professionals. Support medication administration in accordance with care plans and HIPAA regulations to ensure safety and compliance. Prepare nutritious meals tailored to individual dietary needs and assist with meal consumption when necessary. Maintain a clean and safe environment by performing light cleaning duties and ensuring infection control standards are met. Provide patient personal care assistance with compassion, respecting each individual's dignity and preferences. Support patients during medical procedures or appointments by providing transportation assistance or accompanying them as needed. Document care activities meticulously in accordance with facility policies and communicate effectively with clients' families or healthcare teams about progress or concerns. Experience Prior experience working in group homes, assisted living facilities, nursing homes, or home health environments is highly desirable. Demonstrated ability to perform patient observation, monitor vital signs, and assist with medication administration safely. Knowledge of dementia care, Alzheimer's care, memory care, hospice care, and caring for individuals with neurodevelopmental or developmental disabilities is preferred. Experience working with individuals with autism or intellectual disabilities supports your ability to provide specialized care. Familiarity with patient transfer methods such as Hoyer lifts and patient personal transportation assistance enhances safety during mobility support. Certification in CPR and First Aid is advantageous; ongoing training in manual handling and heavy lifting techniques is beneficial for safe caregiving practices. Strong communication skills for client interaction, documentation, and collaboration within multidisciplinary teams are essential for success in this role.
