Job Summary We are seeking compassionate and dedicated Hospice Aides to provide high-quality in-home and community-based care to patients facing end-of-life conditions. The ideal candidate will assist with daily activities, monitor patient well-being, and support families through sensitive care practices. This role offers an opportunity to make a meaningful difference in patients' lives while working within a supportive healthcare team. Responsibilities Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support. Monitor and document patient observations, vital signs, and overall condition using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems. Support medication administration under supervision, ensuring proper documentation in compliance with HIPAA regulations. Provide specialized care for patients with dementia, Alzheimer's disease, or other memory impairments, including memory care techniques. Assist with catheter care, wound care, and other medical procedures as trained. Operate equipment such as Hoyer lifts safely to transfer patients while maintaining patient dignity. Support meal preparation and feeding for patients requiring assistance, ensuring nutritional needs are met. Perform light housekeeping tasks such as laundry and cleaning to maintain a safe and sanitary environment. Collaborate with nursing staff and other healthcare professionals to ensure comprehensive patient care plans are followed. Maintain confidentiality of patient information in accordance with HIPAA standards. Skills Proven experience in hospice care, home health, or long-term care settings preferred. Ability to work with individuals with disabilities or developmental challenges, including dementia or Alzheimer's disease. Proficiency in EMR/EHR systems for accurate record keeping. CPR certification required; additional certifications such as medication administration or catheter care are a plus. Compassionate caregiver with excellent communication skills and patience when working with patients and families. Ability to work independently while following established care protocols and safety procedures. Experience working in assisted living or nursing home environments is advantageous. Physical ability to assist with patient transfers and perform mobility support tasks safely.
