Job Summary

TWELVE37, INC.

is seeking compassionate and dependable individuals or families to become Sponsored Residential Providers for individuals with intellectual and developmental disabilities (IDD). This is not a traditional hourly or shift-based position. A Sponsored Residential Provider welcomes an individual receiving Virginia Medicaid DD Waiver services into the provider's own approved private home and provides ongoing, person-centered residential support. As a Sponsor, you will help the individual become a valued member of your household while providing the supervision, support, transportation, advocacy, skill-building, and assistance necessary for the individual to live safely and successfully in the community. This opportunity is ideal for someone who has a stable home environment and wants to make a meaningful difference while providing long-term, family-style support to an individual with a developmental disability. Responsibilities Sponsored Residential Providers are responsible for providing individualized support based on the person's ISP, Plan for Supports, assessed needs, preferences, and level of supervision.

Responsibilities may include:

Welcome the individual into your private home and provide a safe, stable, clean, and supportive living environment. Provide supervision consistent with the individual's ISP, risk assessments, behavioral needs, and safety requirements. Assist with activities of daily living (ADLs), including bathing, grooming, dressing, toileting, eating, and personal hygiene as needed. Support independent living skills including cooking, cleaning, laundry, grocery shopping, budgeting, and household routines. Administer or assist with medications in accordance with physician orders and required training. Monitor changes in the individual's physical, behavioral, and emotional health. Schedule, transport, and accompany individuals to medical appointments as appropriate. Provide reliable transportation for appointments, shopping, recreation, family visits, religious activities, employment, and community activities. Promote meaningful community participation based on the individual's interests and preferences. Support the development of communication, social, coping, safety, and independent living skills. Implement the individual's ISP, Plan for Supports, behavioral strategies, medical protocols, and risk-management plans. Utilize positive behavioral supports, approved de-escalation techniques, and crisis-prevention strategies when necessary. Protect the individual's privacy, dignity, choice, independence, and Human Rights. Maintain accurate and timely daily documentation, medication records, health records, and other required service documentation. Immediately report injuries, medication errors, medical emergencies, behavioral crises, suspected abuse/neglect/exploitation, elopement, and other significant incidents. Participate in required training, meetings, home visits, inspections, quality reviews, and annual evaluations. Maintain compliance with TWELVE37, DBHDS, Medicaid, HCBS, and applicable Sponsored Residential requirements.

Minimum Qualifications Applicants must:

Be able and willing to provide services within their own private residence . Maintain a safe, clean, stable, and appropriate home environment. Be dependable, patient, compassionate, and committed to supporting individuals with developmental disabilities. Successfully complete required background checks and screenings. Complete all required training and maintain required certifications. Be able to provide appropriate supervision based on the individual's assessed needs. Maintain confidentiality and professional boundaries. Have strong communication and documentation skills. Be able to respond appropriately to medical, behavioral, and other emergencies. Have reliable transportation. Maintain a valid driver's license, automobile insurance, and a safe vehicle when providing transportation. Be willing to participate in home inspections and ongoing monitoring. Be able to work collaboratively with TWELVE37, Support Coordinators, guardians, families, healthcare providers, and other members of the individual's support team. Preferred Experience Previous experience is preferred in areas such as: Intellectual and Developmental Disabilities (IDD) Sponsored Residential Services Direct Support Professional (DSP) services Group homes or residential services Personal care or caregiving Behavioral health Autism services Community-based disability services Medication administration Individuals with strong caregiving experience who are new to Sponsored Residential services are also encouraged to apply.

Please Read Before Applying Sponsored Residential is not a traditional 8-hour shift or hourly caregiving position. The individual receiving services lives in the Sponsor's approved home . Becoming a Sponsor is therefore both a professional and household commitment. The individual should be treated as a member of the household—not simply as a tenant, guest, or client occupying a bedroom. Sponsors are actively responsible for supporting the individual's health, safety, welfare, rights, independence, community participation, and quality of life. Support needs vary by individual. Some individuals may require minimal assistance, while others may require significant supervision, behavioral support, personal care, medication assistance, or help with daily activities.

Pay:

$3,400.00 - $7,500.00 per month
