Job Description

OhioHealth is seeking a dedicated LPN to join our Healthcare & Medical Services team. In this role, you will provide compassionate, patient-centered care including medication administration, basic treatments, patient education, and coordination with RNs and providers. You will document accurately in the EMR, support care plans, and contribute to a safe, inclusive environment for all patients and families. Responsibilities Provide direct, hands-on nursing care to patients under the supervision of RNs and physicians Perform focused patient assessments and monitor vital signs, reporting changes in condition promptly Administer prescribed medications and treatments following Ohio Health policies and scope of practice Document care, assessments, and patient responses accurately in the electronic medical record Assist with diagnostic tests, procedures, and specimen collection as directed Support patient mobility, hygiene, nutrition, and activities of daily living Reinforce patient and family education on care plans, medications, and discharge instructions Collaborate with the interdisciplinary care team to coordinate and optimize patient care Follow infection prevention, safety, and quality protocols to maintain a safe care environment Assist with patient admissions, transfers, and discharges, ensuring continuity of care Provide empathetic, culturally sensitive support to patients and families from diverse backgrounds Participate in ongoing training, competencies, and quality improvement initiatives Required Skills Basic nursing assessment skills Medication administration and dosage verification Vital signs monitoring and interpretation Wound care and dressing changes IV therapy and injections (per licensure and unit policy) Electronic medical record (EMR) documentation Infection prevention and control practices Basic EKG recognition (if applicable to unit) Patient and family education skills Time management and prioritization in a clinical setting Clinical communication and handoff skills Safe patient handling and mobility techniques
