Assesses health and psychological needs and develops an individual wellness plan for the coordination of care services for members. Develops proactive wellness interventions and oversees the implementation of the plan. Key Responsibilities:
Actively participates in member application process by conducting initial health and memory screening to determine program eligibility. Coordinates services for members, including psychological assessments, coordination of services, crisis intervention education and medical decisions related to their health care. Assists with scheduling medical tests, concierge services and transportation arrangements. Performs wellness assessments and goals while visiting with members in their home, and educates members about available services at Sun Health. Recommend and coordinate community resources and interventions that maximize independence and promote safety at home. Provides advocacy and acts as a liaison between members and their family, and follows up with member or family concerns as needed. Follows up with members after hospital or health center admissions. Assists with follow-up coordination of services. Educates and assists with the psychological support during transitions to other levels of care. Provides medical advocacy, ensuring medical providers and services are coordinated to the maximum benefit of the member. Identifies the need and develops support groups for members as appropriate, while acting as an active member of an interdisciplinary team. Performs all functions in assigned areas following established schedules and using prescribed methods. Ensures an atmosphere which allows for the privacy, dignity and well-being of all members in a safe, secure environment. Employs an approach of contact and communication that demonstrates a high level of customer service. Ensures the confidentiality of all members and/or employee-related information. Attends all mandatory in-service training programs and required meetings and takes an active role in performance improvement studies, focus groups, etc. for the betterment of residents. Be available after hours, and on weekends to respond to urgent needs or schedule needs of members. All other duties as assigned by the Executive Director. Your Qualifications:
Registered Nurse or Licensed Practical Nurse License required Minimum one (1) year of related experience in assisting adults with care coordination and care, care management and wellness programming. Proficient reading, writing, grammar and mathematical skills. Proficient in assessing needs for adults and coordinating services, developing / running support groups and developing wellness programs. Proficient interpersonal relations and communication skills, including demonstrated ability in dealing with complex issues and managing people. Working knowledge of community-based services and an understanding of the continuum of care. Negative results for annual tuberculosis testing. Possess current Level 1 Fingerprint Clearance Card or have the ability to obtain a Level 1 Fingerprint Clearance Card required
