Part-Time Join a Team That Makes Every Moment Matter Harmony Community Healthcare is seeking a compassionate and dependable Licensed Practical Nurse (LPN) to join our hospice team providing weekend care to patients in their homes. If you are passionate about delivering exceptional end-of-life care and enjoy working independently while being supported by an experienced interdisciplinary team, we'd love to meet you. Bilingual (English/Spanish) is Required As a Weekend Hospice LPN, you'll provide skilled nursing care under the direction of a Registered Nurse, helping patients remain comfortable while supporting families through one of life's most important journeys. What You'll Do Provide compassionate nursing care to hospice patients in their homes. Carry out the patient's individualized plan of care under the supervision of the RN. Assess patient condition and promptly report changes to the RN and physician. Administer medications and treatments as ordered. Educate patients and families regarding disease processes, medications, comfort measures, and home care. Document visits accurately and timely within the EMR. Participate in interdisciplinary team collaboration to ensure excellent patient care. Maintain compliance with Medicare Conditions of Participation, agency policies, and professional standards. Assist with admissions, routine visits, PRN visits, and symptom management as needed. Communicate effectively with physicians, nurses, social workers, chaplains, aides, and other members of the care team. Qualifications Current, unrestricted Arizona LPN license. Graduate of an accredited Practical Nursing program. Minimum one (1) year of nursing experience preferred. Hospice, home health, long-term care, or palliative care experience is strongly preferred. Current CPR certification. Valid Arizona driver's license, reliable transportation, and proof of automobile insurance. Ability to work independently while exercising sound clinical judgment. Excellent communication and documentation skills. Ability to successfully pass a background check and obtain/maintain an Arizona Fingerprint Clearance Card. Bilingual (English/ Spanish) required Physical Requirements This position requires the ability to: Frequently stand, walk, bend, kneel, and lift. Safely assist with patient care activities. Lift and move equipment and supplies. Travel throughout the Nogales and surrounding area to patient homes. Why Harmony?
