Job Description

Job Overview We are seeking a dedicated and compassionate Licensed Vocational Nurse (LVN) to join our healthcare team. In this vital role, you will provide high-quality nursing care to diverse patient populations across various settings, including long-term care facilities, assisted living communities, hospitals, and home care environments. Your expertise will ensure patients receive personalized, safe, and effective treatment while supporting their overall well-being. Responsibilities Perform comprehensive patient assessments, including vital signs, physical examinations, and health histories to inform care plans. Administer medications and injections accurately following prescribed protocols, including IV infusions and catheter care. Assist with diagnostic evaluations such as specimen collection, processing, and medical imaging procedures. Manage airway support devices like ventilators and provide emergency interventions such as CPR and first aid when necessary. Document all patient interactions thoroughly using Electronic Health Record (EHR) systems like Epic or Cerner, ensuring compliance with HIPAA and confidentiality policies. Collaborate with interdisciplinary teams to develop and implement individualized care plans tailored to each patient's needs, including dementia, Alzheimer's, or memory care cases. Educate patients and their families on health management strategies, medication administration, nutrition, and discharge instructions for home & community care. Requirements Valid state licensure as a Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN). Proven experience working with diverse patient populations in settings such as long-term care facilities, hospitals (including ICU or trauma centers), hospice, or emergency rooms. Strong knowledge of medical terminology, anatomy, physiology, and nursing practices related to geriatrics, pediatrics, acute pain management, infectious disease care, and dementia care. Proficiency in EMR/EHR systems like Epic or Cerner; experience with ICD coding (ICD-9/10) is highly desirable. Ability to perform complex patient assessments and prioritize urgent needs effectively in fast-paced environments. Excellent communication skills for caregiver interaction and client education; experience working with individuals with disabilities is a plus. Certification in Basic Life Support (BLS) and sterile technique; additional certifications such as IV infusion training or ventilator management are advantageous.
