In this rewarding role, you will provide skilled nursing care and health monitoring services for seniors and adults with chronic health conditions, disabilities, and cognitive impairments in a supportive adult day care setting. You will work closely with participants, families, caregivers, and healthcare professionals to promote health, safety, independence, and overall well-being. Responsibilities Conduct participant health assessments, including vital signs and routine health monitoring. Administer medications and treatments according to physician orders and facility policies. Maintain accurate medical records and documentation using EMR/EHR systems. Monitor participants with chronic conditions and report changes in condition to appropriate healthcare providers. Provide wound care, blood glucose monitoring, and other skilled nursing services as needed. Assist in developing and implementing individualized care plans. Support participants with dementia, Alzheimer's disease, and other cognitive or physical limitations. Collaborate with interdisciplinary team members to ensure quality participant care. Educate participants and caregivers on health management and wellness practices. Maintain compliance with HIPAA, infection control, and safety regulations. Qualifications Current California Licensed Vocational Nurse (LVN) license in good standing. Experience in adult day care, senior care, assisted living, long-term care, home health, or related healthcare settings preferred. Knowledge of geriatric care, chronic disease management, and medication administration. Strong assessment, communication, and documentation skills. Experience with EMR/EHR systems preferred.
