At Apex Infusion, we deliver experienced, one-on-one, personalized care for complex conditions in the comfort of your home or in our private Ambulatory Infusion Suites (AIS). We are growing rapidly throughout California and looking for committed professionals who want to make a difference.

Position Overview:

We're seeking a dependable and organized healthcare professional, a Quality Assurance LVN, to join our team in Signal Hill.

Key Responsibilities:

Conduct routine clinical chart audits to ensure compliance with state, federal, and organizational regulations. Review patient records for accuracy, completeness, and proper documentation by clinical staff. Monitor adherence to quality improvement (QI) and quality assurance (QA) programs within the facility. Identify documentation deficiencies and collaborate with staff to implement corrective actions. Assist with regulatory surveys, inspections, and accreditation processes. Track and report quality metrics, incident reports, and performance indicators. Educate clinical staff on documentation standards, compliance policies, and best practices. Participate in root cause analysis for adverse events, incidents, or patient safety concerns. Ensure compliance with infection control protocols, patient safety policies, and clinical standards. Maintain accurate records of quality audits, improvement initiatives, and compliance reports. Collaborate with interdisciplinary teams to improve patient care outcomes and operational quality. Support implementation of performance improvement plans and quality initiatives.

Qualifications:

Must have an associate degree (A.A.), or equivalent from a two (2) year college. Must have at least one year's experience in healthcare or a related field. Must work well with other disciplines to coordinate patient care. Must have a current state driver's license. Experience in reviewing nurses' notes for completeness, compliance with physicians' orders, authorized services, regulations required. Experience in reviewing admission packets required. Experience required in reviewing all POC and re-certification for: accuracy, order updates, frequency updates, goal updates, medication profiles review, format and wording, diagnoses update, and C.N.A. care plan. Effective written and verbal communication.
