Job Description

We are looking for a compassionate and dependable Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join our nursing team. This position is ideal for a nurse who enjoys building relationships with residents, working as part of a supportive team, and providing high-quality care in a skilled nursing and rehabilitation setting. As an RN/LPN, you will play an important role in ensuring our residents receive safe, personalized, and compassionate care while helping create a positive environment for residents, families, and staff. Responsibilities Provide direct nursing care in accordance with resident care plans, physician orders, and facility policies. Assess and monitor residents for changes in condition and communicate concerns promptly. Administer medications and treatments accurately and safely. Complete nursing documentation and maintain accurate resident medical records. Coordinate care with physicians, CNAs, therapy, social services, and other members of the interdisciplinary team. Communicate with residents and families regarding care, treatments, and changes in condition. Provide wound care and other skilled nursing treatments as ordered. Respond appropriately to emergencies and changes in resident condition. Provide compassionate care to residents with dementia, cognitive impairment, and other complex healthcare needs. Supervise and support CNAs while promoting teamwork and quality resident care. Follow infection prevention, safety, and regulatory standards. Advocate for residents' dignity, preferences, independence, and quality of life. Qualifications Current RN or LPN/LVN license in good standing. CPR/BLS certification preferred. Skilled nursing, long-term care, rehabilitation, or similar experience preferred, but new graduates are encouraged to apply . Strong communication and organizational skills. Ability to work effectively as part of an interdisciplinary healthcare team. Dependable, compassionate, and resident-focused.
