Services provided include skilled nursing, physical therapy, occupational therapy, speech therapy, and non-skilled home health aide/companion. HCSF specializes in post-transplant care, advanced wound care, trauma rehabilitation, post CVA rehabilitation, and infusion/chemotherapy.

Qualifications:

We are looking for a per diem responsible LPN with experience in wound care and Insulin Injection to service our clients in a facility and also for home visits. We are looking for a very personable individual who has the ability to foresee any home care needs the patient assigned to them may have. Experience in Home Health (MUST) IV certified (PLUS) Kinnser knowledge (PLUS) Must have all credentials in place including

Background Screening and recent in-services complete
