Job Description
OhioHealth is seeking a dedicated LPN to join our Healthcare & Medical Services team. In this role, you will provide compassionate, patient-centered care including medication administration, basic treatments, patient education, and coordination with RNs and providers. You will document accurately in the EMR, support care plans, and contribute to a safe, inclusive environment for all patients and families. Responsibilities Provide direct patient care within LPN scope under supervision of an RN and/or provider Obtain and document vital signs, patient histories, and ongoing assessments Administer medications and treatments safely following organizational policies and standards Assist with diagnostic tests, procedures, and specimen collection Monitor patients for changes in condition and promptly report concerns to the RN or provider Document all care accurately and timely in the electronic health record (EHR) Reinforce patient and family education on diagnoses, treatments, and discharge instructions Support infection prevention and patient safety protocols at all times Collaborate with the interdisciplinary team to support individualized care plans Maintain a clean, organized, and stocked patient care environment Participate in quality improvement, safety, and competency-based training activities Demonstrate culturally sensitive, respectful communication with patients, families, and colleagues Required Skills Medication administration and reconciliation Vital signs measurement and basic patient assessment Electronic Health Record (EHR) documentation (e.g., Epic or similar) Wound care and dressing changes Phlebotomy and specimen collection IV therapy and infusion management (per scope and certification) Basic life support and emergency response techniques Infection prevention and control practices Patient and family education techniques Time management and prioritization in a fast-paced clinical setting Interdisciplinary team communication and collaboration Understanding of HIPAA and patient privacy regulations
