Responsibilities Conduct comprehensive patient assessments, including vital signs, physical examinations, and medical history documentation using Electronic Health Record (EHR) systems such as Epic or PCC. Administer medications and injections accurately following prescribed orders while adhering to HIPAA and clinical confidentiality policies. Perform specialized procedures such as IV infusion, catheterization, tube feeding, venipuncture, and sterile dressing changes with precision and aseptic technique. Manage acute pain and chronic conditions through appropriate interventions, including medication administration and patient education on pain management strategies. Assist with diagnostic evaluations like specimen collection/processing, medical imaging preparation, and wound suturing when necessary. Provide patient care in various settings such as medical-surgical units, ICU or Level I Trauma Centers, emergency rooms, hospice environments, or long-term care facilities. Communicate effectively with patients and caregivers about treatment plans, medication instructions, and discharge planning to ensure understanding and compliance. Maintain accurate medical records and documentation of all patient interactions in compliance with healthcare regulations such as NCQA standards and long-term care regulations. Support infection control protocols by practicing proper hand hygiene, equipment sterilization, and environmental sanitation to prevent healthcare-associated infections. Collaborate with multidisciplinary teams on case management for patients with complex needs including dementia care, Alzheimer's disease, developmental disabilities, or infectious diseases. Utilize EMR/EHR systems like Athenahealth or eClinicalWorks for documentation; demonstrate proficiency in
ICD-9/ICD-10
coding for billing and reporting purposes. Experience Valid Licensed Practical Nurse (LPN) license with current state licensure required. Prior experience working in diverse healthcare environments such as hospitals (including ICU or trauma centers), skilled nursing facilities, assisted living communities, or home & community care settings preferred. Demonstrated proficiency in critical care experience including ICU or emergency medicine settings is highly valued. Experience working with geriatric patients or individuals with disabilities—especially those requiring dementia or memory care—is essential. Knowledge of advanced nursing procedures such as airway management, ventilator support, dialysis assistance, or cardiac catheterization is advantageous. Familiarity with EMR/EHR systems like Cerner or Athenahealth; experience with ICD coding (ICD-9/ICD-10) enhances efficiency in documentation processes. Strong understanding of long-term care regulations, HIPAA compliance policies, and patient safety standards is required to deliver high-quality care consistently.
