Job Description

OhioHealth is seeking a dedicated LPN to join our Healthcare & Medical Services team. In this role, you will provide compassionate, patient-centered care including medication administration, basic treatments, patient education, and coordination with RNs and providers. You will document accurately in the EMR, support care plans, and contribute to a safe, inclusive environment for all patients and families. Responsibilities Provide direct bedside and clinic-based nursing care under supervision of RNs and providers Perform patient assessments within LPN scope, including vital signs, monitoring, and reporting changes in condition Administer medications, injections, and treatments according to physician orders and Ohio Health protocols Assist with wound care, dressing changes, specimen collection, and basic diagnostic procedures Document all care, assessments, and patient responses accurately in the electronic medical record (EMR) Support patients and families with education about diagnoses, medications, procedures, and discharge instructions Collaborate with physicians, advanced practice providers, RNs, and allied health staff to coordinate safe, efficient care Maintain infection prevention and control standards, including hand hygiene and proper use of PPEContribute to care planning, rounding, and interdisciplinary discussions focused on optimal patient outcomes Assist with patient admissions, transfers, and discharges, ensuring safe handoffs and continuity of care Promote patient safety by following fall prevention, medication safety, and equipment use guidelines Participate in ongoing training, competencies, and quality improvement initiatives within the department Demonstrate cultural sensitivity and uphold Ohio Health's values of compassion, respect, and inclusion in all interactions Required Skills Medication administration and safety Basic patient assessment and monitoring Wound care and dressing changes IV therapy and phlebotomy (per scope and training) Vital signs measurement and interpretation within scope Electronic medical record (EMR) documentation Infection prevention and control practices Patient and family education Basic life support and emergency response support Time management and patient prioritization Interdisciplinary team communication Specimen collection and handling Basic diagnostics support (e.g., EKGs, point-of-care testing) Safe patient handling and mobility assistance Understanding of scope of practice and nursing regulations
