Job Description

POSITION
Licensed Practical Nurse (LPN/LVN)
Classification/Status:
Non-Exempt/Full Time Shifts:
7P-7A Full Time position (2-2-3 rotation - 12 Hour shifts) Reports to: Nurse Practitioner (DON) Summary/Objective The licensed practice nurse (LPN) and/or licensed vocational nurse (LVN) is responsible for rendering nursing care in assigned area in terms of individualized client needs, according to dependent and independent nursing functions and conformance with recognized nursing techniques, procedures, and established standards based on the scope of practical nursing under the direction/delegation of the registered nurse. Utilize nursing knowledge and skills in the safe implementation of basic preventative therapeutic and rehabilitative nursing care of assigned patient/family as evidenced by documentation and observation of positive patient care outcomes. 2. Perform delegated independent nursing functions using established procedures, policies, guidelines and standards as observed by the registered nurse. 3. Administer medication accurately, observing patient response, as evidenced by documentation in the medical record and lack of negative outcomesevidenced within medical record, client and health care team feedback. 4. Assist in assuring that effective communication exists with patient/significant other as reflected in the plan of care. Give punctual change of shift report consisting of pertinent and relevant information for assigned patients. 5. Work collaboratively with other members of the health care team in coordination of the patient's care as evidenced by other department feedback. 6. Use available written and AV materials specific to patient condition or disease. Utilize appropriate teaching methods such as one-on-one and group instruction, demonstration or discussion. 7. Assist patients, family members or other clients with concern and empathy; respect their confidentiality and privacy and communicate with them in a courteous and respectful manner. 8. Answer and refer telephone calls or other inquires to ensure accurate and timely communications are facilitated. Competencies 1. Customer/Client Focus. 2. Ethical Conduct. 3. Personal Effectiveness/Credibility. 4. Thoroughness. 5. Time Management. Supervisory Responsibility This position has no direct supervisory responsibilities. Work Environment This job operates in residential treatment setting. This role requires regular walking around the facility. This role also routinely comes into contact with patients who may have contagious illnesses. Physical Demands While performing the duties of this job, the employee is regularly required to talk and hear. This position is very active and reguires standing, walking, bending, kneeling, and stooping all day. The employee must frequently lift and/or move items over 20 pounds. Expected Hours of Work This is a full-time position in a medically managed residential facility which is open 24 hours a day, 365 days a year. Rotating shifts as well as holiday and work during inclement weather will be required. Travel No travel is expected for this position Required Education and Experience 1. Graduate of an accredited school of Nursing. 2. Valid registration as an LPN/LVN in the state. 3. Current AHA Healthcare Provider training (BCLS) preferred. Pay:
$25.00 - $27.00 per hour
