Duties Perform comprehensive patient assessments, including vital signs, physical examinations, and health history documentation using Electronic Health Record (EHR) systems such as Epic or Cerner. Administer medications and injections accurately following prescribed protocols while maintaining strict adherence to HIPAA and clinical confidentiality policies. Assist with medical procedures such as catheterization, tube feeding, sterile processing, venipuncture, and wound suturing to support optimal patient outcomes. Monitor patients' conditions closely, utilizing skills in airway management, ventilator care, ICU procedures, and acute pain management to respond swiftly to changes in status. Collaborate with physicians and interdisciplinary teams to develop and implement individualized care plans for patients with complex needs like dementia, Alzheimer's disease, or infectious diseases. Maintain meticulous medical records and documentation of patient care activities in compliance with long-term care regulations and ICD coding standards (ICD-9/10). Provide education and support to patients and their families on health coaching, medication management, post-discharge instructions, and caregiver communication strategies. Qualifications Valid state licensure as a Licensed Practical Nurse (LPN) with current certification. Proven experience working in diverse healthcare environments such as long-term care facilities, skilled nursing facilities, hospitals (including trauma centers), or assisted living communities. Demonstrated proficiency with EMR/EHR systems like Epic, Cerner, or Athenahealth; experience with medical imaging or medical documentation is a plus. Strong knowledge of anatomy, physiology, pharmacology related to medication administration, and clinical assessment techniques. Experience working with geriatric patients or individuals with developmental disabilities; familiarity with dementia care and memory care is highly desirable. Ability to perform IV infusion therapy, catheter care, first aid procedures, and basic life support (BLS), CPR certification required. Excellent communication skills for effective caregiver interaction and client education; ability to work well under pressure in fast-paced settings such as emergency rooms or urgent care clinics. Knowledge of infection control practices and long-term care regulations; experience with ICD coding (ICD-9/10) is advantageous.
